The new Z83 form: how to fill it in correctly
A section-by-section guide to the new Z83 application for employment form.
Which Z83 to use
Since 1 January 2021 only the new Z83 form is accepted. Old versions lead to disqualification. You can complete it online with our Z83 tool and print it for signing.
Part A: the advertised post
Write the exact post title, the department and the reference number as they appear in the advert. Indicate your notice period.
Part B: personal details
Complete your ID number, date of birth, race, gender and disability status (used for employment equity). Answer the questions about criminal convictions, disciplinary cases and business with the State honestly — false information can lead to dismissal later.
Parts C to E: contact details, qualifications and experience
Give a phone number and email you check often. Qualifications and experience may be summarised if you attach a detailed CV, but the fields must not be left blank — write "see attached CV" at minimum where the form allows.
Sign and date
Sign the declaration by hand (or with an electronic signature where the department accepts email applications). An unsigned Z83 is invalid.
